Centre of Tekksellence Facility Waiver and Release of Liability

Please read this agreement carefully. Every athlete (or a parent/guardian, for participants under 18) must complete the form below before their first session at Centre of Tekksellence, 10480 105th Drive N, Jupiter, FL 33478.

Acknowledgment and Assumption of Risk

I understand that participation in soccer activities, training sessions, leagues, tournaments, camps, clinics, and use of the Facility's premises and equipment involve inherent risks, including slips, trips, and falls; collisions with other participants, spectators, staff, equipment, or structures; sprains, strains, fractures, concussions, and other injuries; medical emergencies; property damage or loss; and exposure to weather-related conditions for outdoor facilities. I voluntarily choose to participate in these activities and knowingly assume all risks, known and unknown, associated with participation and use of the Facility.

Release and Waiver of Liability

To the fullest extent permitted by law, I, on behalf of myself, my heirs, executors, administrators, representatives, and assigns, release, waive, discharge, and covenant not to sue Centre of Tekksellence, its owners, operators, directors, officers, employees, contractors, coaches, trainers, volunteers, sponsors, affiliates, and agents ("Released Parties") from any and all claims, demands, actions, damages, losses, liabilities, costs, or expenses arising out of or related to personal injury, illness, disability, death, or property loss or damage resulting from my participation in activities at the Facility or use of its premises, equipment, or services, except where caused by gross negligence or willful misconduct of the Released Parties.

Medical Authorization

I certify that I am physically capable of participating in soccer activities and have no medical condition that would prevent safe participation. In the event of an emergency, I authorize the Facility and its representatives to obtain emergency medical treatment on my behalf if I am unable to do so, and I understand that I am solely responsible for any medical expenses incurred.

Facility Rules, Conduct & Property Damage

I agree to follow all Facility rules, policies, and staff instructions; use equipment safely and responsibly; conduct myself in a respectful and sportsmanlike manner; refrain from abusive, threatening, discriminatory, or unsafe behavior; and immediately report any hazardous conditions or injuries. I understand that failure to comply with Facility rules may result in removal from the premises without refund. I also agree to be responsible for any damage to Facility property caused by my intentional misconduct, negligence, or misuse of equipment.

Indemnification

I agree to indemnify and hold harmless the Released Parties from any claims, damages, liabilities, costs, or expenses arising from my actions, conduct, or participation in activities at the Facility.

Photography and Media Release

I authorize the Facility to use photographs, video recordings, or other media containing my likeness for promotional, advertising, marketing, or educational purposes without compensation. You can indicate your consent preference on the form below.

This Agreement is governed by the laws of the State of Florida. If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect.
